MD/ND COLLABORATIVE CARE PROGRAM
We believe in the benefits of an integrative approach to restoring health and maintaining wellness, especially when working with complex chronic illnesses (such as Chronic Fatigue Syndrome, Lyme disease, chronic pain syndromes, autoimmune conditions, and others).
This is why we have created a program which combines the unique skills of both Naturopathic Doctors and Functional Medicine Doctors to fully address your issues in comprehensive, holistic and patient-centered fashion.
Your evaluation starts with an intake questionnaire that can be completed online prior to your first visit. This form may take a few hours of time, please complete this form to the best of your abilities at least 5 days prior to your initial visit, as it will give us time to review your history ahead of time.
At your first visit, you will spend time with both the ND and the MD to take a full history and order investigations as needed to dig deeper into the root causes of your illness.
This visit will also touch on the core tenets of restoring health-nutrition, sleep, movement and stress management.The follow up appointment will be another joint appointment with MD and ND, and we will discuss and explain the results of your tests, answer any questions and initiate a collaborative treatment plan.
Just as no two patients are alike, no two treatment plans are alike-we will work with you to create a treatment plan that suits your individual needs and takes into account the uniqueness of your situation. We use all the tools at our disposal, and may even bring other members of our team into your care plan when indicated.
Included in the program are the following services:
- Review of your Functional Medicine intake form.
- Initial and Follow up appointments with ND
- Integrative/Functional MD fees
- Collaboration and discussion around your case outside of patient visits
Additional costs to be expected:
- Functional Lab testing done through outside labs(such as stool microbiome testing, salivary hormone profiles, advanced tick borne infection testing, urine toxic metals test, etc).We offer these tests at wholesale cost, with a nominal flat fee for lab administrative services, to assure you that there is no conflict of interest when ordering these tests.
- Cost of nutritional supplements and non-prescription medicines as needed.
- Consultations with other members of the Venn Med team when needed.
- Further follow up care with your treating ND.
